Manchester United midfielder is set for a spell on the sidelines after damaging his knee ligaments playing for Belgium on international duty.
Fellaini was taken from the field after 29 minutes of Belgium’s 4-3 World Cup qualifier win over Bosnia in Sarajevo, in Group H, which the Belgians have won at a canter.
United already have Paul Pogba and Michael Carrick sidelied, meaning Fellaini's comes as a huge blow for manager Jose Mourinho, who will now have to readjust his 4-2-3-1 formation, or adapt another one of his squad to fill Fellaini's box-to-box role just ahead of the flat back-four.
Belgium boss Roberto Martinez said of the injury: "We are all worried about Fellaini.
"We fear that there is ligament damage."
An official for the Belgium team also commented: "It is something to be worried about on first sight. Ligaments, left knee. More news [on Sunday]."
The Belgian FA subsequently announced on Twitter that Fellaini would miss several weeks with what is believed to be medial ligament damage, meaning he could miss all of United's upcoming matches against Liverpool, Tottenham, Benfica and Chelsea.
